For All Applicants (Signed by Applicant, and/or by Guardians for Minors under Age 18) registration is NOT completed without submitting the ONLINE REGISTRATION APPLICATION FORM (which is provided as a part of the Online Registration Order Confirmation) and which includes:

• ADULT OR PARENTAL CONSENT (for Minors), INSURANCE, MEDICAL TREATMENT AUTHORIZATION CLAUSE
• RELEASE OF LIABILITY AND ASSUMPTION OF THE RISK AGREEMENT
• PAL PUBLICITY RELEASE
• PROOF OF APPLICANT AGE (Copy of Birth Certificate or Photo ID with Age)

Additionally for All ADULTS over the Age of 18 (Applicants and/or Guardians of Minors) the ONLINE REGISTRATION APPLICATION includes:

• SAN JOSE PAL ADULT CODE OF ETHICS

And for the Junior Giants and Senior Giants Programs Only it includes:

• Junior Giants / Senior Giants Player Waiver
